WHAT YOU'LL DO As a Case Manager you will be responsible for assisting our clients in the different treatment programs offered, primarily in coordinating and providing care that is safe, timely, effective, efficient, equitable, and client-centered.
This position will be providing direct client services in the areas of soft skills training, employment counseling, client needs assessment, vocational assessment, and employment preparation by providing comprehensive assessment, planning, implementation and overall evaluation of individual patient needs. The Case Manager serves as liaison with community resource agencies to coordinate services needed.
